Online applications are invited from intending eligible Indian citizen as defined in Article 5 to 8 of the Constitution of India for 65 (Sixty- Five) Grade-IV, sanctioned posts of Government of Assam, in various districts of Assam, under the establishment of the Directorate of Health Services; Family Welfare), Assam. Reservation: Reservation of posts for SC/ST (P)/ST (H), OBC, Women, Differently Abled candidates will be as per prevailing Government Rules.

• Educational Qualification: Class VIII Passed
• Age: Candidate should not be less than 18 years and not above 44 years as on 01.01.2018. The upper age limit is relaxable by 5 years for SC, ST (P), and ST (H) Candidates only.

• Uploading of E-Admit Card, rejection list, and accepted list will be informed subsequently via website and local newspapers. Applicants are advised to keep track of the website and newspaper for any updates.
• The following documents in original shall have to be produced at the time of document verification. Scanned copies of the following should be uploaded in the online portal:
• (a) Scanned copy of recent passport size colour photograph.
• (b) Scanned copy of signature. The following documents must be scanned and self-attested and then uploaded
• (c) Proof of age: Birth Certificate or certificate from Head Master from a School in Assam.
• (d) HSLC certificate/Class VIII pass certificate from Head Master of school in Assam. (e) Caste Certificate (where applicable) from competent authority
• (f) Permanent Residential Certificate of Assam or Domicile Certificate from competent authority.
• (g) Employment Exchange Registration Certificate.
• If more than one candidate secures the same final marks the candidate senior in age shall be placed higher in the select list. The vacancies are notified with the administrative approval of Government of Assam. The undersigned reserves the right to rectify/modify the advertisement.
• Selection process:

The selection process will be consist of OMR based written test. Schedule and other details of written test shall be uploaded at www.dhsfw.assam.gov.in in due time.
